The addition of a nurse practitioner will provide essential support to caregivers following acute care admissions. |
| Bethany Care Society | Funding will support the RESPECT Connect program, which works to give family caregivers what they need: clear, compassionate, and timely information to make confident decisions for their loved one. By transforming how prognosis is shared in long-term care and identifying effective communication strategies, this program reduces uncertainty and fear by fostering understanding and preparedness, empowering caregivers to advocate effectively, plan, and focus on what matters most: the dignity and quality of life of their loved one. |
| Gaia Community Care and Wellness Society | Funding will support the REACH (Respite, Education, Advocacy, Care and Holistic Support) Caregivers program, which is a culturally and linguistically responsive support initiative that empowers family caregivers, especially from Chinese-speaking and immigrant communities, through coordinated, meaningful and compassionate care. Many caregivers experience burnout, isolation, and difficulty navigating healthcare systems, especially when language and cultural barriers exist. By combining respite, education, advocacy, and emotional care, this program aims to restore confidence in caregivers who are navigating the complex health systems. |
| Fraser Valley Health Care Foundation | Funding supports the Holding Space: Empowering Family Caregivers in Mental Health and Aging Support program, which is an initiative designed to uplift, educate, and empower those family caregivers who are supporting someone with psychological health challenges, or age-related conditions, like dementia. This initiative addresses an important gap for spouses, adult children, and aging partners who shoulder the weight of mental health and care at home. By offering practical tools, emotional support, and system navigation tools, the program transforms isolation into connection. |
| Thompson Seniors Community Resource Council Inc. | Funding will support the Respite Case Study program, which works to fill a gap that is currently preventing advocacy for respite care within Manitoba. Despite growing numbers of older adults, Indigenous elders, and newcomer families living with chronic and life-limiting conditions, limited data exists to demonstrate the scope of caregiver needs. This program will invest in evidence that leads to action, ensuring that family caregivers in Manitoba are seen, supported, and have access to the resources they need. |
| Capital Region Mental Health & Addictions Association, Inc. | Funding will support the Caregiver Support and Resource Development program, which will strengthen the mental wellness of families by expanding resources, programs, and peer support so the care recipient and their family caregivers can have a safe space to learn, connect, and heal together. Funding will provide caregivers with the tools required to care for themselves and the mental health of their loved one. |
| Seniors Resource Centre Association of Newfoundland & Labrador Inc. | Funding will support the Empowering Caregivers: A Community-Based Elder Mediation Model for Newfoundland and Labrador program, which works to provide family caregivers the support they need to navigate complex and isolating decisions related to aging. Although elder mediation is proven to resolve disputes constructively, access to this support remains limited, and inconsistent. This program, in partnership with Family Mediation, will address this gap through raising public awareness, building local capacity through training mediators within community partner organizations, and implementing a real-time evaluation model to assess the outcomes of the services. |
| Caregivers Nova Scotia Association | Funding will support the CAN-CARE Project, which works to support family caregivers who experience intimate partner violence while providing care. These caregivers face unique and compounding vulnerabilities that few programs in Canada currently address, and through this program, Caregivers Nova Scotia and its research partners will fill the gap in Canada's caregiving and family violence response system. Funding will allow Caregivers Nova Scotia to create the first national framework that ensures caregivers living in unsafe or abusive relationships are not left to choose between their own safety and their loved one's care. |
| Hospice Halifax | Funding will support the Caregiver Grief and Bereavement Counselling programs, which provide flexible counselling options for family caregivers who cannot attend sessions during regular working hours. This free program supports caregivers who are coping with a loved one's terminal diagnosis or death through one-on-one counselling group sessions, and virtual options, each specifically tailored to caregiver needs across Nova Scotia. With a part-time social worker offering evening and weekend virtual counselling sessions, caregivers can access grief support while managing daily responsibilities. |
| ALS Canada | Funding for ALS Canada will support the one-on-one Therapy Services program for caregivers across Canada. This program invests in the mental health of family caregivers, through trained professionals focusing on the individual needs of those in a caregiving role. The service would provide specialized disease-informed mental health support to caregivers, offering them the space, tools, and professional guidance they need to cope with trauma, grief, and exhaustion. |
| Body Brave | Funding for Body Brave will support its initiative to address the gap in Canada's eating disorder care system by supporting family caregivers who facilitate recovery. Despite being central to healing, caregivers are often excluded from treatment pathways and left without the tools and confidence needed to navigate complex care journeys. This program will give caregivers the knowledge, community and emotional resilience they need to sustain care safely to improve recovery outcomes, reduce burnout, and strengthen the national continuum of eating disorder care. |
| Inclusion Canada | Funding will support the Fueling the Spark of Inclusion: Empowering Family Caregivers Nationwide project, which transforms family caregivers from isolated supporters into empowered leaders of inclusion. Caregivers of people with intellectual disabilities often carry the daily weight of systemic barriers and societal misconceptions. This program removes logistical barriers so caregivers can learn, connect, and lead change in their communities. |
| McMaster University | Funding will support a Social Prescribing program for carer-employees, which provides access to tailored supports and community resources that are unique to those family caregivers who are balancing a role in paid employment and unpaid caregiving. This program pioneers an evidence-based approach to a pressing challenge: the well-being of working caregivers. In partnership with the Canadian Institute for Social Prescribing and the Canadian Red Cross, this project will develop a national model that bridges health, workplace, and community systems. |
| Melanoma Canada | Funding will support the Dedicated Caregiver Support Group, a monthly virtual group created specifically to ensure that family caregivers feel supported as they help their loved ones navigate melanoma. By establishing this project as well as a private online community for caregivers to share and obtain guidance, Melanoma Canada ensures that every caregiver has access to compassionate, professional, and peer-led support. |
| Ontario Caregiving Organization | Funding will support the National Caregiver Collaborative: Mobilizing Research to Address Caregiver Needs program which transforms caregiver research into actionable insights and collective power for systemic change. By uniting the expertise of provincial caregiver organizations, such as Caregivers Alberta and Caregivers Nova Scotia, this program synthesizes and mobilizes evidence to drive better policies, programs, and outcomes for family caregivers nationwide. |
| Partners for Planning | Funding will support the Family Compass: Guiding Care and Connection project, which works to provide hands-on support and community infrastructure that family caregivers of individuals with intellectual and development disabilities urgently need. By offering direct, one-on-one support and strengthening peer-led family networks, this program works to empower caregivers to advocate effectively, plan, and access critical resources. |
| Young Caregivers Association | Funding will support Digitizing a Sustainable Young Caregiver model, a collaboration with Family Caregivers of BC, Caregivers Alberta, and Caregivers of Nova Scotia, which aims to support young caregivers in Canada by using digitization to create a hub for organizations to access resources and train staff/volunteers to support young caregivers. This ensures that no young caregiver is left unseen or unsupported due to geography or funding limitations. This initiative will transform a successful pilot program into a national support network, using digitization to ensure that operations across the country have access on how to start their own young caregiver program; while ensuring it is based on expert knowledge and quality curriculum. |
| University of Ottawa Heart Institute | Funding will support the Empowering Caregiver Partnerships in Cardiovascular Disease Research project, which works to fill a critical gap in cardiovascular research by meaningfully engaging family caregivers. The program includes developing relevant, accessible, and caregiver educational resources on common research methods, including what it means to be a caregiver partner, as well as developing a full-day workshop for caregivers to share their stories, and learn about current research on caregiving. The projects and its activities will follow a community-based participatory research design, so that caregivers can get increased representation in cardiovascular disease research, as well as a newfound sense of knowledge and confidence. This project will allow Canada's cardiovascular research system to become more representative, innovative, and responsive. |
| Saint Elizabeth Health Care | Funding will support the Respite For All: Expanding Employer Supported Respite for Canada's Working Caregivers project, which works to bridge the gap between working caregivers and family caregiving by bringing structured, workplace-based respite and mental health support to employees – particularly in low-income, rural and equity-seeking communities, who are most at risk of burnout. By building on proven partnerships between SE Health and GreenShield, this model would empower caregiver support into wellness programs, transforming workplaces into sources of relief rather than stress. |
| Palliacco | Funding will support the Respite Care and Psychosocial Support for Caregivers of People with Cancer or at the End-of-Life project, which works to give family caregivers of individuals with cancer and those at the end-of-life rest, relief, and peace of mind. Through the program's in-home respite and psychosocial support, caregivers can take a break knowing their loved ones are in compassionate and competent hands. |
| L'arc En-ciel Regroupement de parents et de personnes handicapées / The Rainbow Association of Parents and People with Disabilities | Funding will support the Supportive Parents – United for Better Support program, which provides family caregivers with the connection and confidence they need to sustain care without losing themselves in the process. This program creates a structured, peer-mentoring network that pairs experienced caregivers with those newly navigating the challenges of disability care. Through training, emotional support, and shared lived experience, this program reduces burnout, breaks down isolation and builds resilience among parents whose caregiving journey often spans decades. |
| Group of Natural Caregivers of Montreal / Regroupement des aidants-naturels de Montréal | Funding will support the Regional Roundtable on Caregivers in Montreal: Montreal Model of Collective Impact and National Outreach project, which elevates family caregivers from invisible supporters to visible changemakers in Quebec. This program brings caregivers and community organizations together to work side by side to influence policy, build knowledge and drive public recognition of caring as a collective social responsibility. By strengthening working committees, producing accessible educational tools and hosting a national caregiving forum, this initiative amplifies caregiver voices, foster collaboration across regions and equips communities to replicate a proven model of participatory engagement. |
| Saskatoon Council on Aging, Inc. | Funding will support the Caring Together: Saskatchewan's Provincial Caregiver Strategy program, which works to expand partnerships, strengthen Indigenous collaboration, and improve access to coordinated resources, for family caregivers and care recipients across Saskatchewan to feel supported and acknowledged. This program works to build a sustainable, province-wide foundation of care for family caregivers, that contributes to a more connected caregiving movement in the community. |

| Alberta Caregivers Association | The funding will be used to support a project led by five Canadian caregiver organizations aimed at improving support for caregivers by addressing overlooked needs such as recognition, transportation, and caregiver support lines. Using the IMPACT Lab framework, the project will foster collaboration and innovation to drive systemic change in caregiving. It builds on past work and involves caregivers at every stage, with the goal of creating a stronger caregiving community across Canada. |
| Wellspring Alberta | The funding will be used to support Wellspring Alberta in providing free, accessible support for individuals affected by cancer, including caregivers, through a variety of programs. These programs focus on six key areas: physical activity, education, symptom management, therapeutic arts, financial/workplace strategies, and emotional support. The funding will also help expand specialized programs for caregivers, children, young adults, and diverse communities, encouraging caregivers to participate in all programs to enhance their wellbeing and resilience. |
| Stigma-Free Society | The funding will be used to expand Stigma-Free Society's youth and rural toolkits with enhanced resources for parents, caregivers, and families, focusing on emotional support through webinars and a docuseries. Key components of the initiative include a Caregiver Self-Assessment Tool, an Expert Tips and Advice Manual developed with mental health professionals, and a Webinar Series addressing caregiving issues. The project also includes Caregiver Stories to inspire caregivers, an Expanded Resource Directory for easy access to support services, and a Mental Health Caregiver Docuseries with positive messages. |
| British Columbia's Children's Hospital Foundation | The funding will be used to support the Sibling Support Centre & Services project, which addresses the lack of support for young siblings of children with chronic health conditions and disabilities in BC, Alberta, and Yukon. The project will provide free online peer support groups for siblings aged 8-11, using a Hub and Spoke model to connect community partners with BC Children's Hospital. Five organizations, including BC Children's Hospital and the Family Support Institute of BC, are involved in the initiative. The program will feature monthly online sessions, in-person events, and tailored curricula. |
| Inclusion New Brunswick Inc | The funding will be used to support the Caregivers Connect Project, which aims to assist caregivers across New Brunswick by providing essential services and ensuring their voices are heard. Caregivers will work one-on-one with an Inclusion NB advocate to address their needs and connect to resources. In addition to offering direct support to 200 caregivers, the project will create opportunities for caregivers to build meaningful relationships and engage in peer mentorship. |
| Caregivers Nova Scotia Association | The funding will be used to revise, redesign, and modernize the Caregiver's Handbook, a key resource for unpaid family and friend caregivers in Nova Scotia. This update will include adding new and relevant information, as well as resources specifically for underserved populations such as working caregivers, young caregivers, newcomers, and the LGBTQIA+ community. The handbook will also be rewritten using plain, inclusive language to improve accessibility. By addressing these needs, the funding will help enhance the caregiving experience across Nova Scotia and ensure comprehensive support for a wide range of caregivers. |
| Inclusion Nova Scotia Society | The funding will be used to expand the EmpowerCare project from its pilot in Cape Breton across mainland Nova Scotia. This initiative will support families and caregivers of individuals with disabilities through grassroots, localized approach. With the ongoing transformative changes in the disability support sector in Nova Scotia, the project will provide emotional support, educational workshops, resource navigation, and advocacy. By empowering caregivers with the knowledge and tools they need, EmpowerCare aims to enhance the quality of life for families during this critical time. |
| The Governing Council Of The University Of Toronto | The funding will be used to support a Phase 2b study and Phase 3 consensus meeting as part of ongoing research at the University of Toronto. This initiative aims to address the gaps in knowledge regarding the mental health and well-being of unpaid caregivers in Canada, specifically focusing on racial and ethnic minority groups such as South Asian, Black, and East Asian caregivers. The funding will support semi-structured interviews to gather in-depth qualitative data and convene a consensus meeting with key stakeholders to identify policy recommendations, tools, and interventions to improve caregiver support. |
| UHN Foundation | The funding will be used to develop and implement a Virtual Caregiver Assistant to support caregivers during the hospital-to-home transition. This innovative tool will be a web-based application powered by artificial intelligence (AI) and natural language processing, providing caregivers with personalized support, information, and resources tailored to their individual needs. The project team will collaborate with digital developers, data analysts, and content experts to ensure the Virtual Caregiver Assistant offers accurate, vetted information from trusted organizations. The goal is to reduce the burden on caregivers by offering immediate, reliable guidance to help them navigate the challenges of caring for their loved ones after a hospital stay. |
| Western University | The funding will be used to pilot McCormick Mobile, a 6-week, multi-faceted program designed to provide personalized, in-home support to caregivers of individuals living with dementia. Developed with input from caregivers and professionals, the program will offer one-on-one visits from social workers, registered practical nurses, personal support workers, and recreation specialists to assess caregivers' needs and provide tailored education. This includes training on communication strategies, medication management, safe personal care, and meaningful recreational activities. Additionally, caregivers will have access to online educational videos and a training workbook to further support their learning. |
| Lakehead University | The funding will support the enhancement and expansion of the "Walking Alongside Indigenous Peoples who are Seriously Ill: Education for Community Caregivers" curriculum. This project will focus on five key components: first, enhancing the curriculum by incorporating new resources based on feedback from Indigenous communities and the Indigenous Guidance Circle; second, developing a "Train the Trainer" workshop, co-designed with the Indigenous Guidance Circle, to equip Indigenous health and social care providers with the skills to use the curriculum effectively; third, creating an accessible, web-based platform to promote the curriculum's resources; fourth, conducting two half-day workshops in Northern and Southern Ontario to train Indigenous health and social care providers; and fifth, disseminating the enhanced curriculum and resources across Indigenous communities, organizations, and partnering agencies. |
| Holland Bloorview Kids Rehabilitation Hospital Foundation | The funding will be used to support and expand the Client and Family-Centred Care (CFCC) initiative in three key areas: enhancing the Family Leadership Program (FLP) by co-creating quality and safety improvements with families, caregivers, and alumni, and expanding family mentorship programs; expanding the Family Resource Centre to offer more virtual family/caregiver workshops, including mental health and wellness interventions in collaboration with CAMH; and improving the Client and Family Relations process by addressing pandemic-related concerns and introducing de-escalation and conflict resolution eLearning modules for staff. |
| St Michael's Hospital Foundation | The funding will be used to develop and implement a social media-based, community-driven wellness promotion program aimed at supporting BIPOC caregivers. It will support three key phases: Phase 1 will focus on engaging BIPOC caregivers and cultural-linguistic chapter coordinators to identify priority caregiving topics, gathering input from at least 30 caregivers, and creating a report to guide the development of a Caregivers' Curriculum. In Phase 2, the funds will be allocated to design and create online resources (infographics, video scripts, etc.) based on the identified topics, which will undergo expert review before being translated, adapted, and posted on a dedicated Caregivers' Hub website and social media channels. Phase 3 will involve monitoring and evaluating the impact of the resources, collecting feedback from caregivers and advisory board members to ensure the curriculum meets their needs. |
| The Baycrest Centre Foundation | The funding will be used to enhance and expand the Canadian Caregiver Assessment and Resources Tool (C-CART) through four key initiatives: 1) Integrating Artificial Intelligence (AI) to provide personalized, dynamic support for caregivers, including voice input and AI-driven prompts to improve accessibility and user experience. 2) Developing a comprehensive knowledge mobilization strategy to increase awareness and adoption of C-CART, which includes partnerships, training for healthcare providers, creating informative materials, and conducting outreach through digital platforms and events. 3) Translating C-CART into French to broaden its reach and accessibility to a larger portion of the Canadian population. 4) Collaborating with Egale to create tailored resources for the 2SLGBTQI caregiving community, addressing the unique challenges faced by caregivers in this group. These enhancements will help expand the tool's impact, ensuring it serves a wider audience and supports caregivers across Canada. |
| The Ontario Caregiver Organization | The funding will be used to support the "Caregiving in Rural and Remote Communities" initiative, which aims to address the unique challenges faced by rural and remote caregivers in Ontario. The project will include two full-day in-person forums in Northern and Southwestern Ontario, bringing together 30-40 caregivers from these areas to share their experiences and challenges. The forums will be designed to create a safe space for open dialogue, with local community organizations playing a key role in identifying diverse participants. A summary report will be created, capturing key insights, caregiver needs, and potential solutions, which will be used to advocate for systemic changes, including more local programming, policy adjustments, and financial support. |
| Parachute Leaders In Injury Prevention | The funding will support "The Caregiver's Parachute" project, providing family caregivers with essential resources to manage the challenges of caring for loved ones at risk of falls. The project includes four quarterly online Caregiver Conversation Series for shared learning, an online Caregivers' Guide to Fall Prevention with practical tips on preventing falls, and a six-part Caregivers' Parachute Podcast offering ongoing support. Continuous feedback will be gathered to improve and expand the resources, potentially exploring new formats like online courses or apps based on caregiver input. |
| Groupe des Aidants du Sud-Ouest | The funding will be used to develop online training and informational modules aimed at supporting working family caregivers in Quebec. This project will create around ten psychoeducational video capsules, focusing on topics such as managing guilt, balancing caregiving with work, and understanding caregivers' rights under Quebec's labor standards. The modules will be available on-demand for flexible access. |
| Fédération Québécoise Des Sociétés Alzheimer/Federation Of Québec Alzheimer Societies | The funding will be used to address the significant need in the community for better guidance and resources for caregivers of individuals living with neurocognitive disorders, particularly Alzheimer's disease. The project focuses on two critical stages in the caregiver journey: end-of-life care and the post-caregiving period. The initiative will provide training to Alzheimer's Societies to enhance their expertise in these areas, enabling them to offer better support to caregivers. This will help caregivers and professionals alike, increasing their ability to guide and assist families effectively during these challenging times. |
| Alliance des communautés culturelles pour l'égalité en santé et services sociaux (ACCÉSSS) | The funding will be used to raise awareness, inform, train, support, and equip family caregivers from ethnocultural communities who face multiple barriers, leading to exhaustion, depression, and potentially an inability to continue supporting their loved ones. These programs serve as invaluable resources throughout an individual’s cancer journey, and they address specific needs of caregivers, young adults, children within cancer-affected families, and diverse communities, including Indigenous peoples. This inclusive approach welcomes all caregivers to participate in programs, irrespective of formal registration. |
| Alzheimer Society of Alberta and Northwest Territories | With our support, the Alzheimer Society of Alberta and Northwest Territories will continue to expand its offerings of programs that support caregivers in their caregiving journey. They offer this support at no cost to caregivers and include in-person and online services to help remove some of the barriers to accessing support. The programs include First Link, Seeds of Hope, Minds in Motion, caregiver support groups, one-on-one support, and ASANT Café and Dementia Talk. These programs each provide information, education, navigational support and a space for caregivers to come together to learn and support one another. |
| Family Caregiver Society of British Columbia (FCBC) | We are proud to support a program by FCBC that aims to make family and friend caregivers independent clients within the healthcare system. It focuses on providing support related to each caregiver's social determinants of health. This innovative program in Canada has successfully made referrals for emotional and practical support and education through FCBC's programs. The program has seen an increase in referrals from various health settings and providers across the province. Moving forward, the program aims to transform its groundwork into a refined and easy-to-adopt model, embracing the Social Prescribing approach to assess caregivers' needs holistically, including housing, nutrition, and exercise. |
| Kids Brain Health Foundation | Kid’s Brain Health Network and a team of experts co-designed and evaluated an Acceptance and Commitment Training (ACT) group intervention for family caregivers of people with Neurodevelopmental Disabilities. ACT is a mindfulness-based psychological intervention which aims to increase psychological flexibility. They combine a Respite Care Co-op and Digital Online Supports to create a sense of community and mutual support. With our funding, Caregivers can easily post requests for respite on the pla torm, and a CareShare facilitator assists in matching them with available helpers. Additionally, caregivers can access support groups and resources on the platorm and earn rewards for participating in the program. |
